Pristine Dental Lab – Payment Policy and Terms & Conditions

Thank you for choosing Pristine Dental Lab as your trusted partner. We are committed to providing you with high-quality dental restorations and exceptional service. The following outlines our payment policy and terms and conditions. By submitting cases to Pristine Dental Lab, you agree to these terms.
I. Payment Policy

Payment Due Date: All invoices are due and payable within 30 days from the invoice date.

New Accounts: For new accounts, the first 2-3 cases may require payment upon delivery (C.O.D.) or upfront payment before fabrication begins, at the discretion of Pristine Dental Lab.

Payment Methods: We accept payment via major credit cards (Visa, MasterCard, American Express, Discover) and cash

Late Payments:

A late fee of 2% per month will be applied to all outstanding balances over 30 days.

Accounts exceeding 60 days past due may be placed on C.O.D. status.

Accounts exceeding 90 days past due may be subject to collection efforts, and the client will be responsible for all collection costs, including but not limited to attorney fees and court costs.

Returned Checks: A fee of $55.00 will be charged for any returned check. Subsequent payments must be made by certified check or money order.

Disputes: Any discrepancies or disputes regarding an invoice must be reported to Pristine Dental Lab within 10 days of the invoice date. Failure to do so will result in the invoice being considered accurate and due as stated.

II. Terms & Conditions

Case Submission:

All cases must be accompanied by a properly completed prescription form from the prescribing dentist, including all necessary patient information, shade selections, material specifications, and design requests.

Incomplete or unclear prescriptions may delay case processing. We will attempt to contact your office for clarification, but fabrication will not begin until all necessary information is received.

Models, impressions, and bites must be accurate, free of distortions, and properly disinfected prior to shipment to our lab. Cases submitted with compromised materials may result in delays or additional charges for remakes if the original materials are deemed unusable.

Turnaround Times & Rush Service:

Rush services are available upon request and will incur an additional fee.

4-5 Day Rush: Call for fees.

Please contact us in advance to confirm spot availability for rush cases, as not all case types can be rushed and availability is at the discretion of Pristine Dental Lab.

Pristine Dental Lab is not responsible for delays caused by shipping carriers, unforeseen technical issues on the milling machines, or force majeure events.

Our standard turnaround times are listed and attached. These times are estimates and begin the next date of receipt of a complete and accurate case at our lab.

Remakes & Adjustments Policy:

Pristine Dental Lab strives for perfection. We will remake a restoration free of charge if the error was due to our laboratory, provided the original restoration is returned within 30 days of the invoice date.

Reasons for free remake include:
-
Restoration does not match the original prescription.
-
Defective materials or fabrication.
-
Significant shade mismatch (when original shade tab was provided and accurate).

Remakes will incur a charge if the error was due to:
-
Inaccurate impression, model, or bite registration.
-
Incorrect shade selection from the dental office.
-
Insufficient tooth reduction.
-
Altering the original prescription during fabrication.
-
Doctor or patient changing the design, shade, or material after fabrication has begun.
-
Restoration damaged during seating or adjustment by the dental office.
-
Failure to return the original restoration.
-

All remakes must be accompanied by the original restoration and a new, complete prescription form clearly indicating "REMAKE" and the reason for the remake.

If the original restoration is not returned, the full fee will be charged for the remake.

Limited Warranty:
-
Pristine Dental Lab warrants that all restorations will be free from defects in material and workmanship for a period of 5 years from the invoice date, provided the restoration is used under normal conditions. This warranty is limited to the cost of the laboratory fabrication.

This warranty does not cover breakage or failure resulting from accident, trauma, neglect, improper oral hygiene, ill-fitting appliances, or normal wear and tear.

This warranty is void if the restoration has been altered or repaired by anyone other than Pristine Dental Lab.

Limitation of Liability:

Pristine Dental Lab's liability for any defect in the products provided is limited to the cost of the laboratory fabrication. We are not liable for any incidental, consequential, or indirect damages, including but not limited to, chairtime, patient inconvenience, or other expenses incurred by the dental practice.

Governing Law: These terms and conditions shall be governed by and construed in accordance with the laws of the State of Florida.

Amendments: Pristine Dental Lab reserves the right to amend these payment policies and terms and conditions at any time. Updates will be communicated to our clients via email, website, or updated fee schedule.
